What Should You Bring to Your Initial Appointment:
Payment:
Patients are required to submit co-payments, deductibles, and co-insurance at the time of visit. We accept Visa, Mastercard, AMEX, Discover checks and cash.
*Please note HeartFit For Duty goes the extra step for testing and some insurance companies DO NOT cover all lab work in full. Please be advised you may receive a bill for these tests from the lab company. You can talk to your provider at your visit regarding the importance of these tests.
- Please be at the location 5-10 minutes before your appointment time.If you arrive 15 minutes or more after your scheduled appointment time, you will be asked to reschedule.
- During your first appointment, we will be doing FASTING blood work which requires the patient to be FASTING for 10 to 12 hours prior to your appointment.
- Please stay hydrated, drink plenty of water during your fasting period.
- Your first appointment will roughly take 90 minutes, so please plan accordingly. You will get your bloodwork drawn, see the nutritionist and meet with your provider.
Getting Your Results:
- Upon completion of your initial appointment, we will schedule you a follow up appointment, with a provider to review your all results.
- Our focus on quality care and the complexity of your testing, a follow up visit is required.

- Driver's License, Insurance card
- Most recent annual exam
- All medications/supplements you are currently taking.
- Completed New Patient Paperwork
- Any questions, topics or concerns you would like to speak with the nutritionist about
